Pathway Community Living provides long-term housing for physically and medically independent adults. Unlike transitional housing or halfway houses designed for temporary stays, our homes offer structured living in a respectful, substance-free community where residents can enjoy stability, independence, and a place to call home.
Intake Process


Referral & Initial Screening
We receive the referral and complete a brief phone screening to confirm basic eligibility, level of independence, and verify income or payment source. When appropriate, we’ll schedule an in-person assessment.

Approval & Room Matching
Once approved, we’ll match the resident with the best available room based on availability, preferences, and compatibility, then complete the required paperwork and collect the initial payment(s).

Move-In & Orientation
Residents receive a smooth, guided move-in with a brief orientation covering house rules, expectations, and available support to ensure a successful transition.
